WHO publishes first road map for testing new tuberculosis diagnostics, aiming to get better tests to patients faster

In brief
The World Health Organization published its first-ever Guidance on evidence generation for new tuberculosis diagnostics on October 21, 2026. The document tells researchers, test developers and funders how to design and report studies so that WHO can review new TB tests, sample types and testing strategies quickly and recommend the ones that work.
Why WHO wrote it
WHO says evidence for new TB tests has often been insufficient or incomplete. Many studies had methodological limits, left out vulnerable groups who could benefit most, looked only at test accuracy, or said little about feasibility, acceptability, equity or cost. Results were also reported inconsistently, which made them hard to combine. The new guidance covers study design, choosing participants, sample size and reporting, and how to measure outcomes that matter to people, such as cost-effectiveness and fairness of access.
A map of where each test fits
The guidance adds an analytical framework that maps where a diagnostic sits along a patient's care pathway and links that to how a study should be designed and reported. It is the third WHO guidance of this kind, after earlier ones on new TB treatment regimens and on TB preventive treatment. Together with WHO's target product profiles for TB tests, the agency says it should help new diagnostics reach the people who need them faster. "Robust evidence is the foundation of strong WHO recommendations," said Tereza Kasaeva, who directs WHO's department for HIV, tuberculosis, hepatitis and sexually transmitted infections.
VSH take: this is a behind-the-scenes document, but behind-the-scenes rules decide how fast a new test moves from the lab to a clinic. Why it matters
Better evidence rules can shorten the time it takes for faster, cheaper TB tests to be recommended and used in the countries that need them most.
